I attached "Debug2" which is a pcm hammer log from attempting to read the PCM again.

Well, I found an old phone and was able to write my needed changes using lsdroid again. Something isn't playing nice!

Must be something from the write with ls droid that makes it think its in recovery mode? I can try a clone, I just assumed that would get caught up on the same step.
