The only time I've seen anything like this is when the cam timing is out.
In that case it had actually ripped some teeth of and a slipped a few teeth.
One thing that was noticed was that as the engine was cranking every so often there would be a puff of air/fuel out the intake as an inlet valve was open as the piston was coming up.

In the end, an engine is pretty simple.
If you have air, fuel and spark in the right quantities and at the right time it has to run, if one of these is missing or spark is at the really wrong time it won't run.

Sounds like its back to basics.

Does it have good compression and what are the readings
Have you done a leak down test and if so what was the out come.

some basic things we do regularly on the race engines to get a idea of whats going on in side.

Yep ,
Done the Basics a number of times as described in first post compression even all bout 130psi leakdown was between 13-19% which i fell is exceptable considering motor has run for 3yrs. I have just replaced intake gasket plugs again it has made know difference. i am about to pull timing cover off to check timing chain and cam timing . I havent seen a holden v8 jump or strip a timing chain before but at this stage anything is possible

Timing chain fine and in correct position :wall:
