They go by many names, P01, P59, VPW, '0411 etc. Also covering E38 and newer here.

It's not the Os version, it's about the quality of the file that was written or how it was written (i.e. Corruption), or the unit itself.
Nothing relevant in PcmHammer has changed, especially up to the point it's getting.

I have found under multiple conditions where the Os will in fact simulate a Mode20 and send out a Mode60.

I know nothing about your VCI, and I don't believe it's the issue, I do believe the PCM has corruption of some sort.

Have you at least tried the stupid simple things ... like, checking the wiring harness, power source condition.
On the pcm the required pins are 19,20,58,60.

Or even trying it in 1x mode (disable 4x) in the Select Device dialog.

I know, but what the hay, it can't hurt to check off the stupid stuff ...

if you update the OS and that re-writes the paramater block as a recovery flash it probably will fix it - not because of the OS change, but because of flashing in a good parameter block.
Have you read the FAQ? For lots of information and links to significant threads see here: http://pcmhacking.net/forums/viewtopic.php?f=7&t=1396

Gampy & Antus, I changed from the 9381344 and performed a Write Entire op using 12202088 OS and all seems well. Still finishing the tune but the 'Hammer reads and writes the PCM now. I did forget that I have a Nano GM and it, along with my other professional tool, works fine for all operations. 9381344 seems to be at least one of the problem child operating systems. I also tried using v18,19,20 along with 21 and none would read the 9381344 so it seems to be coincidental that my issues began while using v21. Thanks for letting me hijack the thread though. :-)

Gampy, to add, yes I always confirm it is not a hardware issue(power supply, wiring, etc) on my end before ranting on the internet. :-)

Trouble getting started. I am trying to read a P59. I have a HP laptop on Win8. I am using an OBDX Pro VT v3 with a usb cable . Com5. I can do a read properties and seems fine, try to do a read entire and It says is unable to unlock. What am I not doing. I followed Gampys order of initializing on this thread about Jun 24 after I rebooted my laptop. I followed the suggest wiring for a bench harness, except my blue PCM plug did not have a wire in 60 for ground on the PCM so I used pin 1, also listed as a ground. I am using a benchtop power supply at 14V.
