first off you need to warm the car up properly first, do a longer log and get it into closed loop. The 02's look to be working, going by the long term fuel trims it's mostly running way to rich, it's completely off the scale in the first MAF table, so idle would be crap and probably have hesitation to. Try for starters drop base inj rate by 10msec/gram, Is the engine stock, what tune you running? More info..

Its just a stock ecotec running a stock BTSJ bin
yes there is hesitation it feels way worse as a manual

Would a crack in the exhaust manifold do it? ?

yeh any leak that the 02 sensor or MAF can pickup will throw fueling out. Which is what it looks like.

also if your gonna use the old tune go and change the flag Not Select Open Loop Idle Logic to enabled so your in closed loop while idling which will help.
